Thread Lifting Essentials: Cog, Mono, and Screw Threads Compared

You’ve heard us talking about the thread lift - how they breathe new life into skin and offer a surgery-free alternative to the facelift. We’ve answered your questions on how thread lifting can be used to reshape the nose and contour the cheek and jawline. Now, if you're doing some research before your treatment at our Vancouver, Abbotsford or Merritt clinic, you might be wondering: What's the difference between cog, mono, or screw threads? And how do you choose the right type of threads?

What is a Thread Lift?

First, let's understand what a thread lift is. A thread lift uses suture material —super thin, body-friendly threads that gradually dissolve. There are three main types of threads—mono, cog, and screw. They each play a unique role in where and how they're placed under the skin to enhance the transformation process. No matter the type, all these threads have one thing in common: as they dissolve, a biochemical reaction happens. This reaction leads to the creation of new collagen, elastin, and blood vessels in the tissues.

Cog Threads: Barbed Threads

Cog threads are your go-to for lifting sagging tissues. The barbs on these threads act like fish hooks, and they come in different configurations to fit different treatment areas. They work effectively on the chin, neckline, cheeks, jowls, eyes, eyebrows, and can even yield dramatic results on the nose.

Mono Threads: Smooth Threads

Mono threads are known for their smooth texture and are primarily valued for their biochemical properties of enhancing collagen and elastin. They're placed just beneath your skin in a mesh-like pattern, tightening the skin, giving it a lift, and greatly improving its texture. These threads are great for the mid-face, lips, and neck, and are effective in treating acne scars. Their placement is done precisely using a sharp needle tip.

Screw Threads: Corkscrew Threads

Screw threads feature a unique spiral shape that not only gives an immediate lift to the skin but also prevents slippage or migration. These threads provide the benefits of both instant skin tightening and long-term rejuvenation, making them especially effective in areas that need extra volume or lift, such as the nasolabial folds or lips. Additionally, screw threads play a crucial role in the Vermillion Lip Border Treatment, defining the red edges of the lips without the need for fillers.

What is the difference between PDO, PLLA and PCL threads?

We’ve talked about the different types of thread configurations, but what about the material?

Thread lift chart: PDO PLLA PCL

Polydioxanone threads (PDO) are extremely robust but dissolve relatively quickly, usually within 3-12 months. Poly-L-lactic acids (PLLA) represent the solid form of liquid Sculptra®. Although not as strong as PDO, PLLA threads have a more prolonged collagen regeneration effect, dissolving over 36 months. Lastly, Polycaprolactone threads (PCL) take the longest to dissolve, lasting over 48 months.

Beyond their extended dissolving time, PLLA and PCL threads are transparent, making them less visible when placed closer to the skin's surface. However, PLLA and PCL threads have a drawback—they lack the same strength as PDO threads and may break under pressure, causing fixation and aesthetic issues. In general, PLLA and PCL threads tend to be more inflammatory than PDO threads, leading to a slightly prolonged recovery time. However, it's important to note that inflammation is a natural part of the regenerative process that we are trying to achieve.
